2010 QEII Hospital Foundation’s Dream Home Lottery
Rules & Regulations
          

Lottery License #: 288755

There are 10,500 tickets at $100, 3,500 sets of 3 tickets for $250 for a total of 21,000 tickets printed.
There are 775 prizes. Ticket buyers are eligible for all 775 prize draws provided they purchase before any draw deadline.

Draw Dates & Early Bird Cut off Dates:
February 22 Tickets go on sale
March 26 Cut off for the first early bird at midnight
March 30 First Early Bird Draw
April 23 Cut off for second early bird draw at midnight
April 27 Second Early Bird Draw
May 21 Cut off for third early bird draw at midnight
May 25 Third Early Bird Draw
June 29 FINAL DRAW DATE at 6:00 pm at Dream Home

(No personal cheques will be accepted less than 3 business days prior to any draw date)

All winners will be notified in writing by The QEII Hospital Foundation, and in some cases by telephone by QEII Foundation representatives, of what they have won and how to claim their prize. All winner's names will be published in a major newspaper in Grande Prairie (Herald Tribune), and posted on the website at www.qe2dreamhome.ca after all draws have been completed.

 
AGLC Raffle Rules & Regulations
 
1.
Lottery tickets shall not be sold to any persons under the age of 18 years. We understand that we cannot enforce individuals not to place the name of a minor on a Dream Home Ticket. Ticket sellers will advise individuals that they should not place the name of a minor on a ticket due to legal implications that may/ will arise if the minor(s) name is drawn.
2.
Tickets:
Tickets will be sold at $100 each 10,500(White) and 3500 sets of 3 for $250 (Green) for a total of 21,000 tickets printed. No tickets may be taken to be sold outside of Alberta, Canada.
We will begin selling tickets February 23rd, 2010.
 NOTICE ADVISORY: In the event a winning ticket stub has more then one name on it, the licensed charity shall award the prize to only one of the individuals identified on the ticket, (The first name appearing on the ticket) and the licensed charity and A.G.L.C are not responsible for any disputes which may arise between the different individuals whose name appear on the ticket.
 
3.
EARLY BIRDS – 1 - 3
The 1st Early Bird draw will be held on
Ø March 30, 2010 at 5:00pm the prize will be a 42” TV with in-home Theater Sound System valued at $1499
Ø Sales Cut off will be March 26, 2010 at Midnight
The  2nd Early Bird will be held on
Ø April 27, 2010 at 5:00pm the prize will be $5,000 Cash or a Cancun Mexico Trip for 2
Ø 200 – Grande Prairie Stompede Weekend Passes
Ø Sales cut off will be April 23, 2010 at Midnight
The 3rd early Bird will be held on
Ø May 25th 2010 at 5:00pm the prize will be $10,000 Cash or a 13 Night Panama Trans-canal Cruise for 2
Ø 100 Country Fever Weekend Passes          
Ø Sales cut off will be May 21, 2010 at Midnight
Ø Each of the early birds will be awarded as per the listing above
 
All Early birds will be open to the public and take place at the QEII Hospital Foundation Office: 10409 -98th St, Grande Prairie Alberta.
The final draw will be held on Tuesday, June 29th, 2010 at 6:00pm or sooner if all tickets have been sold and approval is granted by AGLC. This draw will be held at the Dream Home with the Dream Home Director of Events and Lottery Officers present. An extension of this date will only be requested should not enough tickets be sold to cover prize costs and expenses.
Ø Sales cut off will be June 29th 2010 at 5:30pm
Payment by cheque will not be accepted less than 3 working days prior to each draw. Cheques will be payable to the QEII Hospital Foundation Dream Home Lottery
 
4.
EXCLUSIONS ON BUYING TICKETS
The QEII Hospital Foundation Board of Trustees Executive Members the Dream Home Lottery Executive Members and the QEII Hospital Foundation Staff, will be prohibited from purchasing lottery tickets, however, family members of these individuals will not be prohibited.
5.
AWARDING PRIZES
The prizes will be awarded in the following order:
Ø Each of the early birds listed above on the date listed above as per point 3
Ø All other prizes will be awarded as per their draw ID number listed in the prize list.
Ø All tickets drawn for each prize will be eligible for subsequent draws and returned to the draw barrel unless when drawing for identical prizes. Winning tickets for identical prizes will be excluded from winning another identical prize. Tickets will be placed back into the draw barrel prior to conducting the draws for other prizes. 
Ø Prize pick up for all prizes will be the sole responsibility of the winner
7.
WINNERS CHOICE
The winner of the Dream Home may choose to take Cash in Lieu, in the amount of $300,000.00. If the winner chooses to take the home, they may assume possession in three (3) weeks time. If the winner has not claimed the Dream Home prize within 30 days of the Dream Home draw date, they automatically win the $300,000 cash option. There will be no other choices in subsequent prizes.
 
8.
WINNING TRIPS
When prize trips are won, the restrictions laid out by the Travel Agency (i.e. dates of travel, time frame, types of accommodations) must be ensued and awarded as is. Reference to restrictions laid out below in 1.4.
 
9.
 
WINNING GIFT CERTIFICATES
Gift certificates for retailers involved in draws shall be used for merchandise in their place of business; no cash will be given. All Gift Certificates will be worth a minimum of $100.
 
10.
 
NEW PRIZES
All prizes are deemed “new” by retailers involved even when and including if used as a demo at the Dream Home. i.e. a hot tub running for months at the home will be the prize awarded and shall be accepted “as is”.
 
11.
 
All prizes except for the Dream Home and Early Birds 2 and 3 must be accepted as awarded with no other cash option.
 
12.
NOTIFYING WINNERS: The QEII Hospital Foundation will attempt to notify the winner of the Dream Home for 30 days from the time of the draw. The winner will be notified by phone and by registered mail. The registered mail will require the signature of the ticket holder as proof of notification. The notification will state that the winner has 5 days to decide on the Dream Home or the $300,000.00 Cash and he/she must appear, in person, to the QEII Hospital Foundation office to make their claim. If the winner has not claimed the Dream Home within 5 days of being notified, they will automatically receive the $300,000.00 cash prize.
The Dream Home will be awarded “as is” with NO exceptions to the winner after it is professionally cleaned and minor repairs have been made (if necessary)  
If the winner cannot be contacted within 30 days, they will automatically receive the $300,000.00 cash prize. The winner then has 1 year from the draw date to claim the $300,000.00 Cash prize. If after 1 year the $300,000.00 cash is not claimed, the QEII Hospital Foundation will request AGLC’s permission to donate the prize to their cause.  
All other winners will have 1 year from the draw date to claim their prize.

Tickets will not be refunded.
 
 
14. EXTENSION
 The QEII Hospital Foundation will request an extension on the Dream Home Lottery if we have not reached our break even point as of the final draw date to take place on June 29, 2010. 
 
15. Claiming Prizes 
  1. The Foundation accepts no liability associated with any loss or damages to individuals or property in any way associated with ANY PRIZE. 
  2. It is the sole responsibility of the winner to claim the prize. If the prize is not claimed within 1 year of the draw date, from the Foundation Office, the QEII Hospital Foundation office will request the permission of AGLC to donate the prize back to our office and use it for another event.  
  3. All prizes must be claimed by showing the winning portion of the ticket &/ or government issued photo identification.  
  4. The QEII Hospital Foundation will not be responsible for the delivery of any of the prizes, excluding travel vacations as booking arrangements must be made directly through Marlin Travel. All large item prizes such as vehicles, boats, quads, sleds, bbq’s & canoe package must be claimed at the Dream Home location.
  5. If a request is made to mail PRIZES to the Winner, the QEII Hospital Foundation is in no way responsible for the item, if it is lost in the mail. It is the winner’s responsibility to pay for the shipping of any item and it is their responsibility to arrange shipping. The Foundation will not solicit sponsors for a replacement prize.  
  6. In order for a prize to be mailed to a winner, they will first have to fax or e-mail a copy of a Prize Release form to the QEII Hospital Foundation office.They will be required to sign a release form referring to the points outlined in this document  
  7. In the event of a dispute the terms and conditions of this document will be the governing decision to resolve the issue.  
 
GIFT CERTIFICATE USAGE/ TRIP RESTRICTIONS/ VEHICLE MAINTENANCE PRIZES & EVENT PASS GUIDELINES
 
Attached information directed from Marlin Travel / Nash Tours (for trip vouchers) which is to be taken in conjunction with any terms and conditions outlined by the QE II Hospital Foundations requirements; that have been put forth in this document. 
    1. There is no cash value to the Trip / Gift Certificate, Event Pass or Vehicle Maintenance Package.  
    2. Gift Certificates, Trips, Event Passes, and Vehicle Maintenance prizes are all non transferable. Should they be transferred to another party the QEII Hospital Foundation Dream Home Lottery will not be held responsible in any capacity for the terms of their redemption or loss. All prizes are subject to the conditions set out by the individual business supplying the prize and are detailed in this document.  
    3. The trip is valid 1 year from the draw date.  
    4. The winner is fully responsible for the coordination of the Trip with Marlin Travel by Nash Tours. Bookings are honored at the discretion and the availability of the booking company. Certain black out periods may apply depending on the weather; as these trips can be influenced by the climate.  
    5. Gift certificates that are issued from businesses that do not deal with travel cannot be redeemed for cash. The winners will be provided with a document listing these terms and conditions with gift certificates  
    6. Gift certificates issued to the QEII Hospital Foundation are valid for 1 year from the draw date.
